Angel Touch Mobile Phlebotomy, LLC – Terms & Conditions (Updated Version)

Welcome to Angel Touch Mobile Phlebotomy, LLC (“Angel Touch,” “we,” “our,” or “us”). By scheduling or receiving services from Angel Touch, you (“Client,” “Patient,” or “you”) agree to the following Terms and Conditions. Please read carefully.

1. Scope of Services

  • Angel Touch Mobile Phlebotomy, LLC is responsible for the drawing, processing, handling, and transporting of laboratory specimens.

  • Angel Touch does not collect any specimens without a valid physician’s order or a request from a laboratory.

  • A valid laboratory script (lab order) must be submitted to Angel Touch prior to the scheduled appointment to ensure proper collection and avoid delays.

  • Angel Touch is not authorized to provide medical advice, diagnosis, or treatment. Any questions regarding your health, test results, or treatment must be directed to your licensed healthcare provider.

2. Laboratory Deliveries & Courier Services

  • Angel Touch ensures the delivery of all collected samples to the appropriate laboratory.

  • If a laboratory requires pickup by a third‑party courier, Angel Touch is not responsible for delays caused by courier services, weather conditions, or any unforeseen issues that occur after the specimen has been delivered or picked up.

3. Laboratory Errors

  • Angel Touch is not responsible for any errors that occur at the laboratory.

  • In the event of a laboratory error, Angel Touch will recollect the sample at a discounted rate.

  • If an error is made by Angel Touch during the collection, handling, or transport process, Angel Touch will recollect the specimen at no additional charge.

4. Billing & Payment

  • Payment for Angel Touch services is required prior to the scheduled appointment date.

  • All billing questions or concerns regarding laboratory testing must be directed to your insurance company or the laboratory billing department.

  • Angel Touch is independent from all laboratories, physician offices, and healthcare facilities.

  • Angel Touch does not set laboratory testing prices and is not authorized to provide discounts for laboratory services.

5. Booking & Cancellations

  • Cancellations made 24 hours in advance will incur a $25 cancellation fee.

  • Same‑day cancellations are non‑refundable.

  • We appreciate your understanding and cooperation.

6. Client Responsibility

  • Clients must provide valid physician orders or laboratory requests prior to specimen collection.

  • Clients are responsible for ensuring the accuracy of their personal, insurance, and laboratory information at the time of service.

7. Gender Reveal Testing Policy

Angel Touch partners with Peekaboo for early gender reveal testing. Angel Touch’s role is limited strictly to the collection and handling of the blood specimen.

  • Angel Touch is not responsible for the accuracy, interpretation, or outcome of Peekaboo laboratory reports.

  • All gender determination results are generated solely by the Peekaboo laboratory.

  • Angel Touch does not guarantee or verify the accuracy of gender reveal results and is not liable for any decisions made based on those results.

  • Any questions regarding test accuracy, report findings, or laboratory methodology must be directed to Peekaboo.

8. DNA Testing Conduct Policy

To protect staff safety and maintain chain‑of‑custody integrity:

  • Arguing, fighting, hostility, or disruptive behavior of any kind is strictly prohibited during DNA testing appointments.

  • If such behavior occurs, the appointment will be immediately terminated with no refund, and the client may be prohibited from booking future services.

  • All parties must remain calm, cooperative, and respectful throughout the appointment.

9. Limitation of Liability

Angel Touch’s responsibility is limited strictly to the professional collection, processing, handling, and transportation of laboratory specimens.
Angel Touch is not liable for:

  • Laboratory errors

  • Courier delays

  • Medical outcomes or decisions based on laboratory results

10. Amendments

Angel Touch reserves the right to update or modify these Terms and Conditions at any time. Any changes will be effective immediately upon posting the revised version on our website or otherwise providing notice to clients.

Patient Privacy:

The Privacy and Security Act require health care providers and other covered entities to properly manage how patient information is obtained, maintained, and disclosed to keep patients PHI protected and secure. Under no circumstances will Angel Touch give out the client’s personal information to unauthorized personnel. All clients’ information is always kept personal and confidential.  Angel Touch Mobile Phlebotomy, LLC abides by all HIPAA regulations.​
